We serve patients in three primary care clinics, a 25-bed critical-access hospital, and in anUrgent Care and Emergency Department. It is who we are.Job Description: The Cardiac Rehabilitation Nurse/Specialist or Exercise Physiologist implements and supervises a physical exercise program for disease and healthy populations. The employee communicates, organizes, and works cooperatively as a member of the healthcare team to continuously improve the quality of patient care services delivered. The employee serves as a professional resource for the department, the health care team, and the community. Main Responsibilities: 1. Effectively performs specified assessment preceding, during and after exercise, and initiates appropriate intervention to maintain safety of patient. 2. Directs patients in the performance of prescribed exercise while at the facility and instructs patients in home exercise. 3. Communicates the program philosophy and goals of the exercise program to patients and assists patients in establishing appropriate goals of rehabilitation. 4. Implements appropriate teaching/counseling for patient and family members. 5. Communicates effectively with team members to provide quality patient care. 6. Evaluates effectiveness of various interventions and charts progress at appropriate intervals. 7. Obtains appropriate physician referrals, patient information and assesses various referrals to Exercise Physiology with also including Pulmonary Rehabilitation and PAD programs. 9.
