SUNY Upstate Medical University Social Worker - Per Diem in Syracuse, New York

Job Summary:

In this position, the Social Worker will provide per diem support and coverage to the Emergency Departments at Upstate Medical University, including the Downtown Campus, Community Campus, and Golisano Children's Hospital. The Social Worker provides comprehensive psycho-social assessment, management, and support to at-risk patients and families. In addition, the social worker addresses social determinants of health and complex issues, assisting and collaborating to make referrals with multiple community agencies to meet the needs of the patients and families. This position is part of the interdisciplinary team that recommends and delivers appropriate social work intervention and support. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned.

Minimum Qualifications:

M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. *New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Must obtain L.M.S.W. within 12 months of receipt of limited permit.
