SUNY Upstate Medical University Social Worker – Ambulatory and Specialty Clinic Float in Syracuse, New York

Job Summary:

In this position, the Social Worker must float across the Upstate University ambulatory and specialty clinic settings to provide continued coverage and social work services to patients and their families. The Social Worker functions as a consultation and referral service to provide a comprehensive psycho-social assessment, management, and support to at-risk patients and families. In addition, the social worker addresses social determinants of health and complex issues, assisting and collaborating to make referrals with multiple community agencies to meet the needs of the patients and families. This position is part of the interdisciplinary team that recommends and delivers appropriate social work intervention and support. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned. A well-qualified applicant will have qualities of adaptability, flexibility, and ability to build rapport quickly with team members and patients.

Minimum Qualifications:

M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. *New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Monday-Friday, 8:30 am -5:00 pm, occasional on-call/holiday coverage on rotation based on operational needs.
