Waterview Heights Nursing and Rehabilitation is currently seeking a Full-Time Social Worker.

Salary: $45k-60k per year

Duties Include:

• Interviewing participants and their families

• Provide psychosocial assessment and counseling to participant and families.

• Developing specific care plans based on the needs of individual patients.

• Helping participant apply for appropriate social services.

• Providing crisis intervention as needed

• Providing grief and loss counseling and supportive counseling

• Strengthening communications among participant, families, and the program or facility staff

Requirements Include:

• A current/valid bachelor's degree

• Long Term Care exp. is a plus.

• NYS Screen Certification preferred.

• Strong admission and discharge planning experience
