Apicha Community Health Center LINKAGE TO CARE SPECIALIST in New York, New York

Our mission at Apicha Community Health Center is to provide high quality, equitable, whole person and culturally responsive care delivered in an inclusive and welcoming manner. Build and expand on our long tradition of care for AAPI, LGBTQ+ and persons living with HIV.

We are currently seeking a Linkage To Care Specialist in our Manhattan location

The major duties of the position include, but are not limited to: Client Services: Conduct case finding and engagement of new referrals and lost to care clients; Conduct outreach and recruitment activities to the target populations including HIV testing events and other health fair; Identify and recruit populations: chronic disease including HIV/AIDS, mentally ill and substance user (targeting Asian and Pacific Islanders, but not limited to) to facilitate early access to treatment and social services; Link target population to care/services, and engage to retain in care; Represent the agency at various venues to promote agency services; Collaboration and Services: Work closely with other staff of the Department of Client Services to coordinate clients' care; Actively participate in any organizational activities (meetings, events, etc.); Work collaboratively with agency staff from various departments; Working as a part of the Department of Support Services at the quality assurance and continue quality improvement (CQI) tasks. Administrative: Maintain accurate records on the services and activities such as attendance, registration, client’s interactions and other required documentation according to the protocols and standards of APICHA’s Support Services Department in designate database and reporting structures; Collects, prepares, and reports of services and activities completed in a timely manner; Participates internal and external meetings as assigned and appropriate; Other activities as assigned.

Qualifications: High school diploma/GED with 1 year of working experience in a related field required; College degree preferred; Strong socio-cultural identification with the target population or experience in working with target population strongly preferred; fluency in Spanish or one of Asian languages preferred; Appreciation of and commitment to Apicha’s mission and values; Excellent MS Office skills (Outlook, Word, Excel, PowerPoint); Ability to work effectively in both a collaborative as well as individual setting; Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing Apicha CHC; Excellent customer service skills to assist in resolving member complaints; Strong verbal and written communication skills – for both internal and external communications; Availability on some evenings and weekends is required.
