Description Providence Place, a Lifespark Skilled Nursing Community, is looking for dedicated and passionate Social Worker to join our growing team. Providence Place is conveniently located on the bus route, located in southern Minneapolis in the Standish Ericcson neighborhood. Here we provide services for our seniors in a TCU (Transitional Care Unit) Care, Long-Term Care and Memory Care. We have been helping seniors Age Magnificently for over 25 years and are part of Senior Care Communities, a local non-profit supporting senior. If so, we want you to join our team and help seniors Age Magnificently! Our Social Workers become family and impact the lives of our residents who live in a Lifespark Skilled Nursing Community. In this position you will be responsible for the operations of the Social Services department, providing for each resident's social, emotional, and psychological needs and the continuing development of the resident's full potential from admission to discharge. Duties include: Develop a social history, social assessment and care plan which identifies pertinent problems and needs, realistic goals to be accomplished, and the specific action to be taken in resolution of the problems and/or needs upon admission of each new resident. Participate in the pre-admission screening of all potential residents and provide information to residents and families during admission. Orient the resident to the long-term care environment and facilitate adjustment upon placement. Document progress notes which relate to each resident's care plan when necessary. Assist the resident and resident's family in discharge and placement planning. Organize family groups to promote communications, education and support between family members, facility staff, and administration and provide counseling, as needed. Preferred Qualifications: Post Secondary Education with a bachelor's degree in a related field. A minimum of two years in long-term care or acute care is preferred. Must be able to relate professionally, positively, and cooperatively with residents, resident's families, and employees. Must possess advanced computer skills.
