Job Summary and Qualifications

The Behavioral Health Lead Social Worker demonstrates responsibility and accountability in the performance of assessment, reassessment, development and implementation of the patient’s plan of care based on patient/family/significant other social needs and in accordance with organizational/departmental standards with special focus on the discharge planning and continuum of care needs. As a staff member, the Behavioral Health Lead Social Worker can expect to care for adult and geriatric psychiatric patients. The Behavioral Health Lead Social Worker interacts with other members of the healthcare team and demonstrates knowledge of the specific psychosocial conditions and treatment therapies for the patient populations. The Behavioral Health Lead Social Worker provides psycho-social services to patients and their families. This individual Plans, organizes, directs, defines and coordinates clinical services necessary to meet the legal, organizational, medical staff and regulatory agencies guidelines. Provide case management, education, group and individual therapy, and discharge planning. Performs community and educational activities. The Behavioral Health Lead Social Worker must provide clinical supervision to any unlicensed clinicians and also participate in PI, evaluating social work performance.

  • The Behavioral Health Lead Social Worker consistently responds to orders within 24 hours of referral. Completes a psychosocial assessment that includes the social, emotional, cultural and discharge planning needs of the patient and family in a timely manner. Patient information is recorded in the patient’s medical record as per hospital/departmental policies. Communicates with the healthcare team to obtain additional information to meet the patient’s short- and long-term needs.

  • The Behavioral Health Lead Social Worker consistently establishes a discharge plan that is ready for implementation at the time of discharge by reassessing and monitoring the development of the discharge plan to ensure effectiveness of implementation. Revises the plan as needed in a timely manner. Supports and promotes the patient/family/ significant other involvement in the treatment and discharge plan decision making process. Consistently provides accurate information and maintains communication with community patient care providers and funding or third-party payors to ensure continuity of care.

  • The Behavioral Health Lead Social Worker assists with the education of designated health care surrogate/proxy/guardian as outlined in the Florida Baker Act Statute. Assists in the coordination of patient’s court proceedings including arranging for patient transportation to mental health court.

  • The Behavioral Health Lead Social Worker participates in the interdisciplinary treatment team meetings. Effectively communicates discharge plans and/or barriers to discharge with other members of the health care team

  • The Behavioral Health Lead Social Worker complies with departmental policies and procedures related to required legal documentation and reporting, performance improvement initiatives, and other activities as established throughout the year.

  • The Behavioral Health Lead Social Worker facilitates Psychotherapy group sessions. Demonstrates knowledge of group process and is consistent in facilitating psychotherapy groups.

  • The Behavioral Health Lead Social Worker provides individual psychotherapy as appropriate. Manages difficult emotional situations. Channels concerns appropriately and confidentially. Effectively communicates findings with the treatment team.

  • The Behavioral Health Lead Social Worker participates in family conferences as appropriate. Coordinates meeting time and location.

  • The Behavioral Health Lead Social Worker consistently demonstrates knowledge and understanding of human behavior, interacts appropriately and creates emotionally supportive relationship with the patient/family/significant other.

  • Supervisory duties of the Behavioral Health Lead Social Worker are:

  • Provides supervision for the practice of social work for all social work functions

  • Collaborates with Program Director or Nursing Director or Department Director with individual audit functions of social work staff documentation and social work specific competencies

  • Responsible for Program and group curriculum, group schedules and group education for all non-therapeutic group facilitation

  • Provides periodic observation of social work conducting therapeutic group facilitation

What qualifications you will need:

  • Masters Degree

  • At least 1 year of Experience

  • LCSW Psychiatric Experience

  • Current Florida LCSW License and CPR/CPI certifications

  • Board Certified Social Worker (BCSW)
