The department of Neurology has an exciting opportunity for a Full Time Social Worker to work at UHealth. The Social Worker, LCSW works with clients in order to help them deal with issues involving mental and emotional health. This position provides emotional support, mental health evaluations and therapy, and case management services.

  • Responsible for social work services at MD and interdisciplinary FMD, HD, and FMD clinics.

  • Works collaboratively with the interdisciplinary staff to design and implement treatment plans for the patient.

  • Conducts suitability assessments of patients in the routine and interdisciplinary clinics.

  • Provides psycho-social assessment and diagnosis of behavioral disabilities with recommendations to identify emotional, social, and environmental Strengths and problems related to their diagnosis, illness, treatment, and/or life situation.

  • Collaborate with the health care team and involve the patient/family in the development and implementation of plans.

  • Facilitate care planning across an Interdisciplinary care continuum which includes interacting with consulting physicians, third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate treatment plans.

  • Provides information and referral to public or private agencies with appropriate follow-up.

  • Provides information to patients about appropriate insurances and disability process.

  • Recruits and onboards SW interns and students with the assistance of administration and HR.

  • Performs competency reviews of interns.

  • Supervises interns and MSW students.

  • Responsible for completing disability forms, workers compensation, Medicaid waiver forms and other governmental program forms and any other forms as assigned.

  • Completes letters as assigned.

  • Provides information and referrals of other agencies that may offer services for patients

Minimum Qualifications

  • Master’s Degree in Social Work

  • Certification in relevant specialty or field

  • Minimum 1 year of experience
