Catholic Health Services Social Worker-MSW-PD in Farmingdale, New York

Overview Good Shepherd Hospice, a member of Catholic Health is seeking a compassionate, dedicated Social Worker to assist patients receiving hospice care and their families in coping with problems resulting from terminal illness. In doing so, the social worker interviews patients and families to obtain psychosocial history and, in conjunction with Hospice team members, establishes a comprehensive, interdisciplinary plan of care. Prepares and updates social work case records including patient and family assessments, nature of social problems, therapy, intervention and response. Directs patients to designated community agencies or resources as necessary. The ideal candidate will have knowledge of social work theories, therapies and techniques of casework process, as well as principles of public welfare and must possess the ability to work non-judgmentally with patients and families of many cultural, religious and socioeconomic backgrounds and lifestyles. Job Details Minimum Requirements: Master's degree in Social Work from a NY State Department of Education and Council of Social Work Education accredited school required Minimum of 2 years prior clinical experience in the healthcare field, specific to bereavement and counseling services for severely ill or terminally ill patients and their family Current Certification by the New York State Department of Education as a Social Worker Salary Range USD $72,000.00 - USD $90,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
