Description Position Summary/Essential Duties: Provides psychosocial assessment and intervention to assist patients and families with adjustment to health care needs. Effectively communicates within the work team and collaborates with a multi-disciplinary approach. Provides linage with community resources to assist the patient in the transition from hospital to discharge location. Actively involved in professional integration and self-development in the social services field. Demonstrate knowledge of principles of growth and development over the life span and uses this information appropriately when interacting with and treating patients. Initiates and maintains positive relationships with patients/customers. Initiates and maintains positive relationships with coworkers. Takes responsibility for self-development and supports a learning environment. Displays commitment to the mission of the hospital and its values.
