Social Worker, The Office of Decent Affairs, Varied Shift, Social Services

UC Health is hiring a part-time Social Worker for the Office of Decent Affairs for the first and second shift at University of Cincinnati Medical Center.

Social Workers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs that promotes continuity of care, and facilitates transitions to the next level of care. The social worker will provide education, consultation, and serve as a liaison to the healthcare team regarding the patient and families' emotional, psychological, social, financial and developmental aspects of care.

The Office of Decent Affairs Social Worker will support the Medical Centers social services department, delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures.

Cincinnati’s ONLY Level 1 trauma center.

Work with a diverse range of patients to gain experience and knowledge.
