Responsibilities
Supplemental Varied Shift Palliative Care RN Coordinator
This position reports to the Program Manager of Palliative Care and is responsible for planning, developing, implementing, coordinating and evaluating palliative care services in collaboration with the Sacramento area Mercy Hospitals and CommonSpirit, in a manner consistent with the Mission and Philosophy of Dignity Health.
Included in this responsibility is developing and maintaining strong relationships with physicians and hospital staff to deliver palliative care, goals of care discussion and possibly comfort care to seriously ill and dying patients. In addition, this position is responsible for collaborating with related healthcare practitioners, the patients and their significant others in assisting with admissions of appropriate patients to hospice service.
Job Summary:
Maintains palliative care policies and procedures
Coordinates and completes palliative care consults
Knowledge and clinical competency of death and dying process
Collaborates on education and orientation programs
To be successful in this role you will need knowledge and ability of:
Clinical standards for hospice and palliative care practice
Medicare and MediCal regulations
Title 22, NHPCO and Joint Commission standards
Effective interpersonal skills, problem solving, conflict management skills
Excellent oral and written communication skills

Qualifications
Minimum:
Five (5) years RN experience preferably in: Med/Surg, Oncology, Hospice, Geriatrics, Intensive Care and/or Geriatric-Psychiatry
BSN
Current CA RN licensure and AHA BLS
Preferred:
MSN
Certification in Hospice and Palliative Care
Pay Range
$49.20 - $71.34 /hour
