Job Brief

Provides individual counseling to adults with Developmental Disabilities following a referral from other treatment resources such as case manager, MSC, Residential staff, day program staff, or family member.

POSITION SUMMARY:

Maintains on-going collateral contact in accordance with HIPAA regulations. Conducts and facilitates initial social service screening, initial evaluation, treatment plan and subsequent periodical assessments. Ensures that all documentation is completed through EMR. Facilitates communication with all providers (external and internal) to ensure optimum continuity of care.

POSITION DUTIES:

  • Ensures that admission packet is complete.

  • Ensures that initial comprehensive functional assessment is completed within two sessions.

  • Ensures that initial treatment plan is developed together with individual in accordance with identified needs per functional assessment within 30 days.

  • Implements treatment plan as per domains and criteria of functional assessment.

  • Maintains on-going, documented communication with related providers to ensure continuity of care.

  • Submits clinical documents and billing through EMR within 3 business days.

  • Participates in departmental meetings, staff training, and quality assurance activities ensure compliance with regulatory agencies; Medicaid,DOH, OPWDD and corporate standards of operation.

  • Performs all other duties as assigned.

REQUIREMENTS:

  • Graduate of an accredited Social Work School with a Master’s Degree; NYS Licensed practice Social Work; and have the Clinical “C” designation status.

  • Two years experience in working with persons with developmental and/or psychiatric disabilities.

  • Personal Computer proficient with Microsoft Word.

  • Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
